  • Good Moral Certificate from the Principal, Guidance Counselor or Adviser
  • Secure Referral Slip from the University Medical Clinic for the Chest X-Ray
  • Latest Original copy of Birth Certificate from N.S.O.

    FOR FOREIGN STUDENTS APPLICANT

  • Entrance Examination Result
  • Scholastic Records
  • Transfer Credential Certificate issued by the former school
  • Passport
  • Student Visa
  • Secure Referral Slip from the University Medical Clinic for the Chest X-Ray
  • ACR-ALIEN CERTIFICATE OF REGISTRATION
  • ICR - IMMIGRANT CERTIFICATE OF REGISTRATION
  • Student Developmental Fee

    FOR TRANSFEREE

  • Entrance Examination Result
  • Scholastic Recods
  • Transfer Credential Certificate issued by the former school
  • Good Moral Certificate from the Principal, Guidance Counselor or Adviser
